Surfside Hole-In-One Golf — Liability Waiver & Release

By participating in Surfside Hole-In-One Golf at The Quarry (3960 Highway Z, New Melle, MO 63385), I acknowledge and agree to the following:

Assumption of Risk: I understand that participation in golf activities involves inherent risks including but not limited to being struck by golf balls or equipment, slipping or falling on the premises, and injury caused by the actions of other guests. I voluntarily assume all such risks.

Release & Hold Harmless: I hereby release, waive, discharge, and covenant not to sue The Quarry, its owners, operators, employees, agents, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury sustained while participating in golf activities at this facility.

Golf Balls & Equipment: I understand that golf balls are projectiles that can cause serious injury. I accept full responsibility for the safe and appropriate use of all golf balls and equipment provided.

Governing Law: This agreement shall be governed by the laws of the State of Missouri.
